Symphony Health — effective November 6, 2026.
This notice describes how medical information about you may be used and disclosed and how you can access that information. Please read it carefully.
This notice applies to Symphony Health’s patient care at its San Ramon and Fremont offices. It describes medical-record privacy. For website cookies, contact requests and analytics, see our separate Website Privacy Policy.
You may request access to, or a paper or electronic copy of, records we maintain about you. Contact your office to arrange a written request and confirm your identity. California generally requires inspection within five working days and copies within 15 days after a written request, subject to applicable exceptions. Only legally permitted fees may apply. If access is restricted, we will explain the applicable reason and review rights.
You may ask us to amend information you believe is inaccurate or incomplete. Explain the correction in writing. If we deny an amendment, we will provide a written explanation within the applicable period, generally 60 days under HIPAA, and explain your right to submit a statement of disagreement.
You may ask us to contact you at a different address or telephone number or by an alternative method. We will accommodate reasonable requests.
You may request restrictions on information used or disclosed for treatment, payment or health-care operations. We generally do not have to agree unless the law requires it. If you pay in full out of pocket for an item or service, you may request that information about it not be disclosed to your health plan for payment or operations, unless disclosure is required by law.
You may request an accounting of certain disclosures during the previous six years. This does not include every disclosure; treatment, payment, operations and other legally excluded disclosures are generally omitted. One accounting in a 12-month period is free; a permitted fee may apply to additional requests. You may request a paper copy of this notice even if you received it electronically.
A legally authorized personal representative may exercise applicable rights for you. We will verify the representative’s authority before releasing information or acting on the request.
Subject to applicable law, we may use or disclose information to provide and coordinate treatment, obtain payment and carry out health-care operations such as administration and quality improvement. For example, information may be shared with another treating clinician or a health plan processing a claim.
Other disclosures may be permitted or required for public-health reporting; preventing a serious threat; reporting abuse or neglect; health oversight; legally authorized research; workers’ compensation; organ donation; medical examiners or funeral directors; certain government or law-enforcement functions; or lawful court and administrative proceedings. Each category has legal conditions and limits. A request alone does not automatically authorize release.
We follow applicable California confidentiality laws when they provide greater protection than HIPAA. Special restrictions may apply to sensitive records, including mental-health, HIV-related and substance-use-disorder information. We obtain authorization or meet the applicable legal exception before disclosure.
You may tell us whether information may be shared with family, friends or others involved in your care or payment. If you cannot express a preference, legally permitted sharing may occur based on professional judgment and your interests, or to address a serious threat. Disaster-relief disclosures are also subject to legal limits.
Written authorization is generally required for marketing, sale of protected health information, most disclosures of psychotherapy notes and other uses not allowed by this notice or law. You may revoke an authorization in writing, except to the extent action has already been taken in reliance on it or another legal exception applies. If fundraising communications occur, you may opt out.
If we hold substance-use-disorder records protected by 42 CFR Part 2, those records cannot be used or disclosed in civil, criminal, administrative or legislative investigations or proceedings against you without your written consent or a court order and subpoena meeting the applicable requirements. If Part 2 information is used for fundraising, advance notice and a choice about receiving those communications are required.
We are required to protect the privacy and security of protected health information, follow this notice and provide a copy. We must notify affected individuals of breaches as required by law. Uses or disclosures outside this notice require your authorization unless the law otherwise permits or requires them.
We may update this notice as permitted by law. Revised terms may apply to information already maintained as well as information received later. The current notice will be available from the office and on this website.
